What is Mallet Finger?

Mallet finger (also called "baseball finger" or "drop finger") is an injury to the tendon that straightens the fingertip (extensor tendon). When this tendon is damaged or pulled off the bone, the fingertip droops and cannot be straightened actively.

The injury typically occurs when a ball or hard object strikes the extended fingertip, forcing it to bend suddenly. This can tear the tendon or pull off a small piece of bone where the tendon attaches (avulsion fracture).

Types of mallet finger:

  • Tendinous mallet: Tendon rupture without fracture – most common type
  • Bony mallet: Small avulsion fracture at the tendon attachment
  • Fracture-subluxation: Large fracture fragment with joint instability – requires surgery

Symptoms

Typical Presentation

• Drooping fingertip – cannot straighten last joint

• Pain and swelling at the DIP joint

• Can passively straighten with other hand

• History of injury – ball strike, caught on object

When to Seek Urgent Care

• Open wound at the fingertip

• Significant deformity or joint instability

• Numbness or change in finger color

• Delay in treatment beyond 2 weeks reduces success

Important: Even if the injury seems minor, seek evaluation promptly. Proper splinting started early gives the best chance of full recovery. Delayed or improper treatment can result in permanent drooping.

Treatment Options

Splinting (First-Line Treatment)

The mainstay of treatment for most mallet fingers:

  • Stack splint or custom mallet splint keeps DIP joint straight
  • Must be worn continuously for 6-8 weeks – 24/7 with no exceptions
  • Even briefly bending the finger restarts the healing process
  • Night splinting continues for another 4-6 weeks
  • Success rate: 80-90% when compliant with splinting protocol

Key to success: Never let the fingertip bend during the splinting period. Even when changing the splint, support the finger on a flat surface to keep it straight.

Surgical Treatment

Surgery is indicated for:

  • Bony mallet with fracture involving >30% of the joint surface
  • Joint subluxation (instability)
  • Failed splinting after adequate trial
  • Open injuries requiring repair

Surgical options:

  • Extension block pinning: K-wire holds joint in extension while fracture heals
  • Open reduction and fixation: For large fracture fragments
  • Tendon repair: For certain tendon injuries

Frequently Asked Questions

What is mallet finger?+

Mallet finger is an injury to the extensor tendon at the fingertip that causes the finger to droop and not straighten at the last joint (DIP joint). It commonly occurs when a ball or hard object strikes the fingertip, forcing it to bend suddenly.

Can mallet finger heal without surgery?+

Yes, most mallet finger injuries heal with splinting alone. The finger is kept straight in a special splint for 6-8 weeks continuously. Surgery is only needed for fracture-mallet injuries involving more than 30% of the joint surface or if splinting fails.

How long do I need to wear a mallet finger splint?+

Continuous splinting for 6-8 weeks is essential, followed by 4-6 weeks of night splinting. The finger must remain straight 24/7 during the initial period – even briefly bending the finger restarts the healing clock.

What happens if mallet finger is not treated?+

Untreated mallet finger can lead to a permanent drooping fingertip and may progress to a "swan neck deformity" where the middle joint hyperextends while the fingertip remains bent. Early treatment gives the best results.

How much does mallet finger treatment cost in Hyderabad?+

Splint treatment for mallet finger costs ₹3,000-5,000 including the splint and follow-up visits. Surgery (when needed) costs approximately ₹35,000-55,000 including K-wire fixation or tendon repair.
