What is Dupuytren's Contracture?
Dupuytren's contracture (pronounced "doo-pwee-TRANZ") is a condition where the connective tissue (fascia) under the skin of the palm gradually thickens and tightens. This process creates nodules and cords that slowly pull one or more fingers into a permanently bent position toward the palm.
The condition is named after Baron Guillaume Dupuytren, the French surgeon who first described the operation to correct it in 1831. It's sometimes called "Viking Disease" because of its prevalence in people of Northern European descent.
Dupuytren's contracture typically affects the ring finger and little finger, though any finger can be involved. It usually develops gradually over years and is most common in men over 50. While the exact cause is unknown, there's a strong genetic component.
Symptoms and Stages
• Firm nodules appear in palm
• May cause mild discomfort
• No finger bending yet
• Observation usually sufficient
• Cords form from nodules
• Fingers begin to bend
• Can't lay hand flat
• Treatment may be indicated
• Severe finger contracture
• Finger bent >60 degrees
• Significant functional loss
• Surgery recommended
Tabletop Test
Place your hand flat on a table. If you cannot flatten your palm and fingers against the surface, you likely have Dupuytren's contracture that may benefit from treatment. This simple test helps determine when intervention is needed.
Treatment Options
For early nodules without significant contracture, watchful waiting is appropriate. Regular monitoring ensures treatment is started when needed.
- Office procedure using needle to divide contracted cords
- Done under local anaesthesia
- Quick recovery, but higher recurrence (50-60%)
- Best for elderly patients or those preferring minimal intervention
- Cost: ₹15,000 - ₹25,000
Limited Fasciectomy: Removal of the diseased fascia only
- Gold standard surgical treatment
- Lower recurrence rate (10-30% over 5 years)
- Performed under regional or general anaesthesia
- Recovery: 4-6 weeks for hand therapy
Dermofasciectomy: Removal of fascia and overlying skin, with skin graft
- For severe or recurrent cases
- Lowest recurrence rate
- Longer healing time

Frequently Asked Questions
What is Dupuytren's contracture?+
Dupuytren's contracture is a progressive condition where thickened tissue (fascia) in the palm forms nodules and cords, gradually pulling fingers into a bent position. It most commonly affects the ring and little fingers and is often hereditary.
Is Dupuytren's contracture painful?+
Usually no. Dupuytren's is typically painless, though some patients notice mild discomfort from the nodules. The main problem is functional – inability to straighten the fingers, which affects daily activities like putting hands in pockets or wearing gloves.
How much does Dupuytren's surgery cost in Hyderabad?+
Dupuytren's fasciectomy surgery in Hyderabad costs approximately ₹50,000 to ₹90,000 depending on the number of fingers involved and complexity. Needle aponeurotomy is less expensive (₹15,000-25,000) but has higher recurrence.
When should Dupuytren's contracture be treated?+
Treatment is recommended when you can no longer place your hand flat on a table (positive "tabletop test") or when contracture exceeds 30 degrees. Early nodules without contracture are usually observed. Don't wait until fingers are severely bent.
Will Dupuytren's come back after surgery?+
Recurrence rates vary: 10-30% after fasciectomy over 5 years, higher with needle aponeurotomy (50-60%). Younger patients with aggressive disease have higher recurrence. Dr. Reddy discusses the best approach for your specific case.
